Position: Physician Assistant / Nurse Practitioner Location: Yuma, AZ. Compensation: $135,000.00 – $155,000.00 per year (base salary plus monthly productivity bonuses). Employment type: Full‑time. UpToDate Subscription : Latest medical info at your fingertips. Essential Functions & Responsibilities Rounding and managing the clinical care of residents at the assigned SNF. Providing high‑quality, patient‑centered comprehensive primary care services to residents. Daily rounding and prescribing medications and treatments for residents. Timely documentation of all patient care/encounters. Available for phone/secure‑messaging consultations with facility nursing staff. Documentation completed in Theoria’s proprietary electronic health record (EHR) system, ChartEasy™ (or PointClickCare, MatrixCare, American HealthTech). Expected to be on‑site on and on time for in‑person scheduled shifts; reliable transportation required. Timely attendance is imperative. Shift Structure Rounding time 7 AM‑1 PM local time; provider must be available to nursing staff and address acute/emergent issues. Shifts begin around 7 AM local time with flexibility within reason. Shifts are daily Monday‑Friday. Charting/documentation can be completed off‑site. This is an in‑person position. Compensation and Benefits Salary based with monthly productivity bonuses.
