- Assess patients across inpatient or outpatient settings and develop tailored treatment plans
- Deliver physical therapy interventions that target mobility, strength, balance, and functionality
- Collaborate effectively with nurses, physicians, case managers, and various rehabilitation professionals to maximize patient outcomes
- Inform patients and their families regarding therapeutic objectives, discharge planning strategies, and home exercise routines
- Maintain thorough records of evaluations, treatments, and patient progress in accordance with clinical policies and regulatory standards
- Engage in multidisciplinary care team discussions and interdisciplinary meetings to ensure seamless patient care
- Doctorate in Physical Therapy (DPT) or equivalent qualification from an accredited institution
- Current state licensure as a Physical Therapist
- Preferred CPR certification
- Specialized certifications (such as geriatric or neurological) are advantageous but not mandatory
- Prior experience is highly preferred
- Adeptness in treating patients with intricate medical and mobility challenges
- Excellent communication and collaborative skills
- Familiarity with digital charting and clinical documentation tools
