Position Summary
The Home Health Aide provides personal care and support to patients in their homes or care settings. Responsibilities include assisting with hygiene, daily living tasks, safety monitoring, and household chores as defined in individualized care plans. This role promotes comfort, hygiene, safety, and emotional well-being while maintaining patient confidentiality and adhering to all organizational and regulatory standards.

PRN as need for 3 p.m. - 11 p.m. shift.


St. Augustine, FL

Additional opportunities: in Duval, St. John's, Clay, Nassau counties as needed
