LPN Care Coordinator

Responsibilities Serve as a liaison promoting coordination, communication, and collaboration among Brookdale teams, residents, families, and healthcare providers within Brookdale’s HealthPlus communities. Uphold open and collaborative communication channels and align with residents’ changing healthcare needs. Collaborate with primary and specialty care providers to formulate resident‑specific care plans, ensuring delivery of medically necessary services. Act as a resident advocate, identifying and improving service delivery, closing gaps in care, and understanding key interventions and care protocols. Review current population’s medical diagnoses to identify care needs and assist in assessing the health, functional, and psycho‑social status of residents. Coordinate services to enhance optimum wellness and manage chronic conditions. Collaborate with hospice, palliative, and home health services/therapy as indicated. Participate in collaborative care reviews (CCR) and care conferences. Educate and explain Brookdale’s care pathways and provide advanced care planning education to residents and families, ensuring each resident has a completed advanced care plan. Provide resources for residents and families to make informed decisions about healthcare needs and champion the benefits of residing and receiving care in the community. Apply quality improvement methodology to analyze, enhance, and manage outcomes, striving to achieve quality measure goals. Communicate promptly to the appropriate care provider regarding changes in residents’ health or well‑being, personal needs, risk management issues, customer service issues, or concerns of family or outside providers. Document all required information as per Brookdale or state regulatory agencies. Qualifications Graduate from an accredited LPN program and have successfully passed the state licensing test. Knowledge of value‑based care models is preferred. Experience working in SNF and HH environments is preferred. Previous experience in care management, case management, or care coordination is preferred. LPN license required. Physical Demands and Working Conditions Standing, walking, sitting, using hands and fingers to handle or feel, reaching with hands and arms, stooping, kneeling, crouching, or crawling. Ability to lift up to 50 pounds. Vision, hearing, talking, tasting, smelling as required. Interaction with co‑workers, residents, or vendors. Occasional weekend, evening, or night work if needed to ensure shift coverage. On‑call on an as‑needed basis. Possible exposure to communicable diseases, infections, latex, blood‑borne pathogens, various drugs, chemicals, infectious or biological hazards. Potential injury from falls, burns, odors, or cuts from equipment; possible exposure to injury while transferring, repositioning, or lifting residents. Requires frequent travel and driving (self‑driven, Tier 2). Benefits Medical, dental, and vision insurance; 401(k).
