About the Role:

The RN Case Manager plays a crucial role in coordinating patient care and ensuring that individuals receive the appropriate services to meet their health needs. This position involves assessing patient requirements, developing care plans, and collaborating with healthcare providers to facilitate effective treatment. The RN Case Manager will also monitor patient progress and adjust care plans as necessary to achieve optimal health outcomes. By acting as a liaison between patients, families, and healthcare teams, the RN Case Manager ensures that all parties are informed and engaged in the care process. Ultimately, the goal of this role is to enhance the quality of care and improve patient satisfaction through effective case management.

Minimum Qualifications:

• Current Registered Nurse (RN) license in the state of practice.
• Bachelor's degree in Nursing (BSN) or equivalent experience.
• Minimum of 2 years of clinical nursing experience, preferably in case management or a related field.

Preferred Qualifications:

• Certification in Case Management (CCM) or similar credential.
• Experience with electronic health records (EHR) and case management software.
• Knowledge of community resources and healthcare systems.

Responsibilities:

• Conduct comprehensive assessments of patients' health needs and develop individualized care plans.
• Coordinate with healthcare providers, specialists, and community resources to implement care plans effectively.
• Monitor patient progress and adjust care plans based on ongoing assessments and feedback.
• Educate patients and their families about health conditions, treatment options, and available resources.
• Document all interactions and updates in patient records to ensure continuity of care.

Skills:

Monday through Friday with an on-call weekend rotation.
