Key Responsibilities\n

• Conduct detailed psychosocial assessments of seniors, analyzing emotional, mental, and social health to develop individualized care plans.

• Work collaboratively with clients and families to outline therapeutic goals and social services tailored to enhance well-being and quality of life.

• Provide ongoing counseling support, guiding seniors through life transitions such as loss, illness, and relocation challenges, while ensuring their voices remain heard in their care.

• Advocate on behalf of clients to access vital community resources and services, navigating issues such as healthcare, housing, and social support.

• Regularly monitor clients' health and safety, utilizing virtual check-ins and in-person visits where necessary to assess progress and maintain engagement.

• Engage with interdisciplinary teams, including nurses and healthcare providers, to devise comprehensive care strategies that promote overall wellness.

• Keep meticulous records of client interactions, treatment plans, and overall progress in compliance with state regulations and ethical standards.

Qualifications\n

• Master's degree in Social Work from an accredited institution.

• Current and valid state licensure as a Licensed Clinical Social Worker (LCSW).
