- Perform in-depth patient evaluations, order necessary diagnostic tests, and create personalized treatment plans
- Independently diagnose and manage acute, episodic, or chronic conditions in partnership with physicians as needed
- Educate patients and their families about wellness, preventative health, and ongoing health management
- Prescribe appropriate medications and therapies as per your practice scope
- Engage in follow-up activities and chronic care coordination
- Work collaboratively with multidisciplinary teams to deliver comprehensive, integrated care
- Maintain detailed and accurate records in electronic medical systems
- Master of Science in Nursing (MSN) or Doctor of Nursing Practice (DNP)
- Graduation from an accredited Nurse Practitioner training program
- Valid Nurse Practitioner licensure in California
- National certification as a Family Nurse Practitioner (FNP-C, AANP or ANCC), or a relevant specialty
- Basic Life Support (BLS) and Advanced Cardiovascular Life Support (ACLS) certifications (or ability to attain within 90 days)
- Exceptional clinical judgment paired with a warm and caring approach
- Strong communication abilities with patients, families, and healthcare teams
- Flexibility in fast-paced, patient-oriented environments
- Proficient in EMR systems and comfortable using telehealth technologies
