Hospice Registered Nurse Case Manager
St Croix Hospice
St. Croix Hospice is seeking a compassionate Registered Nurse Case Manager to provide 1:1, whole-person care to patients and families in their homes. You will coordinate with an integrated care team and support families through complex decisions. The role requires RN licensure and at least 1 year of acute care experience; hospice/home health experience is preferred. Responsibilities include assessments, care-plan development, medication administration, patient education, and on-call coverage. #J-18808-Ljbffr
