Resident Care Coordinator (RCC) – RN/LPN

Silvis Center | Silvis, IL

What You’ll Do:

Coordinate resident care plans and ensure quality nursing care.
Support and guide nursing staff to promote excellent resident outcomes.
Monitor resident conditions, documentation, and regulatory compliance.
Collaborate with physicians, families, and the interdisciplinary team.

What We’re Looking For:

Active Illinois RN or LPN license.
Long-term care or skilled nursing experience preferred.
Strong leadership, communication, and organizational skills.
A passion for resident-centered care.
