Position Overview Title: Clinical Social Worker, CF Location: Pleasant Valley State Prison, Coalinga, CA, Fresno County. Classification: This classification is subject to Section 5, Hiring-Above-Minimum (HAM) Authorization of the California State Civil Service Pay Scales. Authorized HAM Rates: P – $8,678, Q – $9,319. Job Description and Duties Under the general direction of the Supervising Psychiatric Social Worker, CF, the Clinical Social Worker (Health/CF) Safety conducts assessments, summarizes case information, diagnoses, collaborates in diagnosis formulation, develops, monitors, and modifies treatment plans with the interdisciplinary treatment team, provides assessments, group therapy, collateral conferences, and documents and evaluates patients’ progress in treatment. Responsibilities include: conducting comprehensive clinical assessments; summarizing case information for diagnosis and treatment planning; collaborating with interdisciplinary team; developing and revising treatment plans; delivering individual and group therapy; facilitating collateral conferences; documenting patient progress; evaluating treatment outcomes; participating in discharge planning; ensuring adherence to agency policies and procedures related to post-release planning, reentry and community safety.
