Responsibilities

• Perform comprehensive occupational therapy evaluations and re-evaluations for residents requiring rehab services.
• Develop, implement, and monitor individualized treatment plans to improve ADLs, mobility, and functional independence.
• Provide direct patient care and educate residents and families on therapeutic exercises, adaptive equipment, and activity modifications.
• Collaborate with an interdisciplinary team (nursing, physical therapy, speech therapy, social work) to support coordinated care and discharge planning.
• Document progress notes, outcome measures, and therapeutic rationale in the charting system in a timely, compliant manner.
• Contribute to care planning and quality improvement initiatives in the long-term care setting.

Requirements

• Active Occupational Therapist license in Connecticut or eligibility for CT licensure
• Master's or Doctorate degree in Occupational Therapy
• 1 year of OT experience in acute care or skilled nursing/long-term care settings preferred
• Ability to work effectively in a multidisciplinary team within a long-term care environment
• Strong communication, documentation, and clinical reasoning skills
