What You'll Do:

• Provide direct physical therapy services to home health clients in their residences
• Collaborate with interdisciplinary team members to develop and implement patient care plans
• Assess patient needs, create customized treatment plans, and modify as needed
• Utilize rehabilitation equipment such as splints, orthoses, prostheses, wheelchairs, ADL equipment, and modalities
• Document patient progress accurately and in compliance with agency policies
• Educate patients and caregivers on home exercise programs and safety
• Ensure safety and quality standards are maintained during patient interactions

What You Need:

• Valid state license as a Physical Therapist
• Home Health Certified (required)
• Experience in home health setting preferred
• Strong interpersonal and assessment skills
• Ability to work independently in a home environment
