Job Summary:

Responsibilities:

Provide personalized care services to clients, including personal care, assistance with daily living activities, and support with mobility and transfers
Conduct vital signs and report any changes to the nurse or supervisor
Assist with medication management, including administration and documentation
Collaborate with the healthcare team to develop and implement individualized care plans
Maintain accurate and detailed records of client care and services provided
Communicate effectively with clients, families, and healthcare providers to ensure seamless care coordination
Participate in in-service training and education to enhance skills and knowledge

Working Conditions:

Work in clients' homes, with varying schedules and environments
May be required to lift, transfer, or reposition clients, and move equipment or supplies
Must be able to work independently and as part of a team
Must be able to effectively communicate with clients, families, and healthcare providers
