Social Services Practitioner I- Per DiemThe County of Riverside Temporary Assignment Program is seeking a Social Services Practitioner I- Per Diem to support the Office of Aging Department located in Riverside. The incumbent in the position will provide initial client assessment through telephone contact and communicate with other Social Services Practitioner staff to carry out client service plans. The incumbent will perform home visits with clients to provide further assessment and application assistance for Social Services Programs. Competitive candidates will have experience working with adult or homeless populations, can work independently, and are proficient in Microsoft Office Suite. Bilingual in Spanish is required and will be required to pass bilingual exam. Meet the Team! The Riverside County Office on Aging is authorized by the Older Americans Act and the Older Californians Act. The department is charged with developing a system of care that offers safety net services for vulnerable older persons and adults with disabilities.Examples Of Essential DutiesProvide case management services to Riverside County residents age 60 and older who are requesting assistance with service connections, community resources, benefits, or stabilization services to support their ability to remain safely and independently in their homes.Conduct home visits and maintain regular telephone contact with clients to assess needs and provide ongoing support.Assist clients with accessing community resources and applying for benefits and services, including Medi-Cal, CalFresh, IHSS, and Social Security.Assist clients with accessing available services through their health plans and coordinating referrals when appropriate.Screen clients for eligibility and access to internal agency services.Provide service navigation, advocacy, and resource coordination based on individual client needs.Complete accurate and timely data entry, documentation, and case organizationMinimum QualificationsEducation: Graduation from an accredited college or university with a bachelor's degree in social welfare, social or behavioral sciences, social/human services, counseling, or a closely related field to the assignment. Knowledge of: socioeconomic conditions and trends; basic principles of individual and group behavior; general trends and current issues in the field of social services; principles of interviewing and problem solving methodology; methods of identification, analysis, and resolution of social casework problems; social service research methods; principles and techniques of social work, and the functions of public social services agencies; basic public social services programs available at the state and local levels; general principles of public assistance policies and programs; laws, rules, and regulations governing the operation of public social services agencies; specialized aid programs; basic computer skills. Ability to: understand and learn programs, policies, and procedures; obtain and recognize relevant and significant facts; speak and write effectively; establish and maintain effective working relationships; establish and maintain client rapport; input and retrieve data from computerized systems.Other Requirements License: Possession of a valid California Driver's License is necessary at the time of hire and must be maintained throughout employment. County vehicles may be available but are not guaranteed. Employees may be required to use their own vehicle for field travel. Mileage reimbursement is available when using a personal vehicle for County business.
