Responsibilities:

• Provide on-call hospice nursing care to patients in their homes and settings within the coverage area.
• Assess patient conditions, manage medications, and coordinate care plans.
• Communicate effectively with patients, families, and interdisciplinary team members.
• Document patient interactions and care activities accurately and timely.
• Respond promptly to patient needs during on-call hours, ensuring high-quality comfort and symptom management.
