Physical Therapist (PT) – Home Health

We are seeking a compassionate and skilled Physical Therapist to provide high-quality, patient-centered care in the home health setting.

• Evaluate patients and develop individualized plans of care to improve mobility, strength, balance, function, and safety.
• Provide skilled physical therapy interventions in the patient’s home.
• Monitor progress and adjust treatment plans based on patient needs and goals.
• Educate patients and caregivers on exercises, mobility, fall prevention, and safe functional activities.
• Communicate effectively with patients, families, physicians, and the interdisciplinary care team.
• Complete timely, accurate clinical documentation while maintaining compliance with home health and professional standards.
• Help patients maximize independence and safely remain in their homes.

Qualifications: Current PT license in the state of practice, strong clinical and communication skills, reliable transportation, and a commitment to compassionate, patient-centered care.
