Ambulatory Care Management Specialist—Referral Coordinator

Barnes-Jewish Hospital with BJC Health is seeking a Care Management Specialist to work in an ambulatory setting with the chronic disease management team. You will handle referral management, scheduling, and communication with providers, specialists, and patients. Strong communication is essential and Epic experience plus Pharmacy Tech license are a plus.
