Float Case Manager

Humana is seeking a Float Case Manager to provide flexible case management support across teams and markets, ensuring members receive coordinated, personalized care. In this role, you will complete comprehensive assessments, develop and adjust care plans, and coordinate services with providers, pharmacies, and community resources. You will prioritize outreach using data and case management tools, close care gaps, and document all activities accurately. Working in Humana’s member-focused, collaborative culture, you’ll partner with interdisciplinary teams to address clinical and social needs, support quality and compliance standards, and help improve health outcomes at scale.
