Collaborate closely with physicians, nurses, and external providers to remove barriers to care, streamline transitions, and support safe, efficient discharge planning. \n MINIMUM REQUIRED QUALIFICATIONS – \n \n \n TX/compact RN license \n BLS \n 1 year case management experience in an acute hospital setting \n Acute hospital experience during the past year with no more than a 90-day break \n PREFERRED QUALIFICATIONS –\n \n \n CCM \n Meditech & Careport experience \n \n \n LENGTH OF ASSIGNMENT – 13 weeks \n SHIFT / HOURS PER WEEK – 8A-4:30P \n SYSTEMS – Meditech, Careport \n START DATE – 9/21/2026 \n \n Facility Location \n \n Fort Worth, which has been rated as one of “America’s Most Livable Communities” embodies all the amenities of a big city, while still holding on to its traditional small-town feel.
