Hospice Float RN – Flexible Clinical Role (Admissions, Visits, Case Support) Location: Roseville, United States Pay: $90,000 – $130,000 DOE Benefits: Full Benefits Package Schedule: Monday – Friday with on-call and weekend rotation Job Title: Hospice Resource RN – Admissions, Visits & Case Management (RN) What You'll Do Admissions: Conduct hospice evaluations, explain services, complete consents, and initiate care plans. Case Management Support: Step in to assist with patient care, symptom management, and coordination. Visit Coverage: Provide routine, PRN, and urgent visits to maintain continuity of care. Care Coordination: Collaborate with physicians, NPs, social workers, aides, and chaplains. Documentation: Maintain accurate EMR charting and ensure compliance with hospice regulations. What You'll Bring Hospice experience preferred or strong background in ICU, oncology, or palliative care. Confidence in admissions, patient assessments, and family communication. Strong organization skills and ability to adapt in a fast-paced, field-based role. Reliable transportation and willingness to travel throughout the service area.
