• Eligible for state medical licensure and DEA registration

What Minimum Requirements You’ll Need

• MD/DO credentialed from the Indiana Medical Licensing Board obtained prior to hire date or job transfer date.
• Advanced Life Support preferred. Accepted: American Heart Association or American Red Cross.
• Doctor of Medicine (MD) or Doctor of Osteopathy (DO) required.
