From comprehensive training to providing necessary protective equipment, we're committed to creating a secure environment for you as you deliver exceptional care in our clients' homes.Position SummaryAs an After-Hours Hospice RN, you will respond to patient and family needs during overnight hours, providing skilled nursing assessments, symptom management, and supportive care in homes and nursing facilities. You will collaborate closely with the hospice care team to ensure continuity of care and promote comfort, dignity, and quality of life.Key ResponsibilitiesProvide after-hours nursing visits to hospice patients in home and facility settings.Assess and manage symptoms, addressing urgent patient and family needs.Provide education, guidance, and emotional support to patients and caregivers.Coordinate care with the interdisciplinary hospice team.Communicate changes in patient condition and ensure appropriate follow-up. Maintain accurate and timely clinical documentation.Qualifications RequiredCurrent New York State Registered Nurse (RN) license.ASN required. BSN preferred1 year of RN experience requiredValid driver's license and reliable transportation.Strong assessment, communication, and critical thinking skills.PreferredOne to two years of nursing experience in home care.Previous hospice or palliative care experience.Hospice experience is preferred but not required.No Hospice experience? Exempt positions under the Fair Labor Standards Act (FLSA) will be paid within the base salary equivalent of the stated hourly rates.
