Nurse Practitioner, In-Home Visits

Position Overview Signify Health, part of CVS Health®, is seeking a Part‑time Clinician (Nurse Practitioner or Physician Assistant) to provide In‑Home Health Evaluations. The role involves conducting 45‑minute visits in patients’ homes, reviewing medication and medical history, performing a physical evaluation, and ordering additional diagnostic tests as directed by the health plan. The clinician uses an iPad and a clinical workflow tool but is not required to prescribe medication or alter treatment regimens. Responsibilities Conduct in‑home health evaluations lasting about 45 minutes. Review medication and medical history. Perform physical examinations and basic diagnostic tests such as spirometry, diabetic eye exams, bone density screening, etc. Use an iPad and an intuitive clinical workflow system for each visit. Provide services as indicated, including diagnostic and preventive products. Conduct virtual visits as needed. Travel locally and within licensed states; flexible to travel as required. Maintain accurate documentation and follow clinical protocols. Qualifications Active, unrestricted license (or licenses) in coverage area(s). Board certification (required). Multi‑state licenses (preferred). Eligible to cover primary location: Hartford, CT. Candidate must be a Nurse Practitioner or Physician Assistant.
