Community Health Navigator & Care Coordinator

Emergemarket is seeking a Community Health Worker to serve as the essential link between patients and all other care providers. The role focuses on removing barriers to medication adherence and continuous care within a harm-reduction framework across community settings. You will engage patients, coordinate with medical and behavioral health teams, manage caseloads, document care in EMR within 24 hours, and advocate for underserved populations.
