PHYSICAL THERAPIST

Responsibilities and DutiesPerforms initial patient evaluations and provides routine Physical Therapy services to patients according to prescribed plan of treatment by a physician.Assess and treat patients with physical therapy through use of physical modalities.Formulates treatment plans based upon individual patient need.Assist persons who are physically challenged to improve their function and mobility and daily living activities.Instructs patients and family teaching in the home.Initiates appropriate discharge planning when necessary.Participates in weekly patient case conference meetings and any staff meetings as notified.Communicates with office staff, physicians, other therapists, etc.Serves as a resource to agency staff regarding physical care of patients.All patient evaluation documentation shall be completed no later than 48 hours from time of completion of evaluation.All care visit notes to be completed within 24 hours from time of completion of visits.WellSky software used for all patient EMRs.Participates in any Performance Improvement Projects and Quality Improvement as needed.Must be currently licensed in the State of Oklahoma as a Physical TherapistMinimum of 1 year of clinical and/or professional experience as a licensed Physical Therapist.Medicare Home Health experience is preferred. #J-18808-Ljbffr
