RN Home Health - Winston Salem, NC - Local or Traveler

Protouch Staffing is seeking a highly skilled and compassionate Registered Nurse (RN) to provide expert, one-on-one nursing care to patients in their homes throughout the Winston-Salem area. This role focuses on case management, patient education, and utilizing strong clinical skills to help patients recover and manage chronic conditions in a non-hospital setting. This position is open to both local and traveling RNs.

Responsibilities:

• Provide skilled nursing care (e.g., wound care, IVs) to patients in their homes.
• Perform comprehensive patient assessments (including OASIS).
• Develop and manage individualized care plans.
• Administer medications and monitor patient health status.
• Educate patients and families on self-care and disease management.
• Document all visits and patient progress accurately in the EHR.
• Coordinate care with doctors and the interdisciplinary team.
• Participate in required on-call rotations.

Qualifications:

• Current RN license in North Carolina (or a Compact State).
• Minimum 1 year of RN experience.
• Prior experience in Medicare-certified Home Health is strongly preferred.
• Proficiency in OASIS and Electronic Health Records (EHR) documentation.
• Current BLS/CPR certification.
• Valid driver's license and reliable transportation.
