Home Therapies Dialysis Nurse

Satellite Healthcare is seeking a compassionate and skilled Home Therapies Dialysis Nurse to join our Healthcare & Medical Services team, supporting patients who receive dialysis treatments in the comfort of their homes. As a key clinical member in a mission-driven, non-profit organization dedicated to individualized kidney care, you will help patients safely manage their dialysis therapies while promoting independence and a higher quality of life. In this role, you will provide comprehensive training and ongoing clinical support to patients and their care partners on home dialysis modalities, including peritoneal dialysis and home hemodialysis. You will conduct detailed nursing assessments, develop and update individualized care plans, and ensure that each patient fully understands their treatment, equipment use, and infection-prevention practices. Your work will include a combination of in-center education, telehealth follow-up, and in-home visits, allowing you to build strong, trusting relationships with patients and families over time. Drawing on your clinical expertise, you will monitor patient progress, recognize and respond to changes in condition, and collaborate closely with nephrologists, social workers, dietitians, and other members of the interdisciplinary care team. You will triage patient concerns by phone or remote monitoring systems, troubleshoot equipment and treatment issues, and elevate clinical problems when needed. Accurate and timely documentation in the electronic medical record and adherence to regulatory and quality standards are essential aspects of your day-to-day practice. As a Home Therapies Dialysis Nurse at Satellite Healthcare, you will have the opportunity to contribute to innovative renal care initiatives, participate in quality improvement activities, and help implement best practices that enhance patient outcomes and experiences. You will serve as a patient advocate and educator, empowering individuals and their caregivers to take an active role in managing chronic kidney disease and home dialysis therapies. This position typically follows a regular full-time schedule with some flexibility to accommodate patient needs, including occasional evenings or weekends. Work is primarily conducted in patients’ homes and outpatient settings, with some responsibilities performed remotely via telehealth and electronic communication tools.
