Home Health Registered Nurse or Licensed Practical Nurse

About the Role We are seeking a compassionate, dependable, and skilled Registered Nurse (RN) or Licensed Practical Nurse (LPN) to provide in‑home patient care near Hunters Point, AZ . The role involves delivering high‑quality nursing services in accordance with individualized plans of care. Employment Type This is a 1099 independent contractor opportunity with an average of 8 hours of nursing care per month, offering a flexible schedule. Employment Note Employment is contingent upon successful completion of a background check. Responsibilities Execute physician‑prescribed plans of care. Complete required documentation in the system of record. Administer prescribed medication, treatment, and therapies. Conduct patient assessments and coordinate care. Perform activities associated with daily living. Monitor vitals and GI intake and output. Assess skin integrity and administer wound care. Monitor for changes in the patient’s condition such as weight changes, self‑care abilities, and disease progression. Communicate with families, physicians, case managers, and other prescribed individuals regarding patient health. Provide education and training to the patient, the family, and the Home Health Aide. Work one‑on‑one in the patient’s home to create a comfortable environment. Collaborate with and supervise Certified Nursing Assistants and Personal Care Attendants. Provide other personal care services as necessary to meet the patient’s needs. Minimum Qualifications Graduate of an accredited college or university; Associate or Baccalaureate School of Nursing. Current state or compact RN or LPN license. Current CPR certification. Valid TB test and physical examination. Required liability insurance. Good interpersonal skills. Hand‑eye coordination and manual dexterity. Ability to stand or walk for prolonged periods, lift up to 50 lbs, and move patients. Ability to operate under stress and make quick decisions. Reliable transportation and willingness to commute to patients’ homes. Ability to provide basic medical management up to medically fragile end‑of‑life care. Preferred Qualifications At least 1 year of experience as an RN/LPN in a clinical or home‑health setting. Ability to assist with two‑person lifts.
