We are seeking a dedicated Physical Therapist (PT) for a full-time or PRN position, offering a competitive wage range of $43 - $55 depending on experience. New graduates are encouraged to apply!

Work Environment and Patient Demographics

Our facilities operate as Skilled Nursing Facilities (SNFs) , providing:

  • Long-term care for residents needing ongoing skilled support and compassionate daily assistance
  • Short-term, sub-acute rehabilitation for patients recovering from surgery, illness, or injury
  • Collaborative care through an interdisciplinary team approach involving nursing, therapy, and medical staff
  • A diverse patient population with orthopedic, neurological, cardiopulmonary, and post-acute needs

As a therapy professional, you will play a crucial role in helping patients achieve functional improvements and enhance their quality of life in a supportive, patient-centered setting.

Position Summary

The Physical Therapist will deliver direct rehabilitation care in accordance with physician referrals, adhering to all relevant laws, regulations, and organizational standards.

Qualifications and Requirements
  • Graduation from an accredited physical therapy program (BSPT, MSPT, or DPT; those graduating in 2003 or later must hold an MSPT or DPT)
  • Current registration/licensure in the applicable state, with an active license in good standing throughout employment
  • Preferred one year of experience; however, new graduates are welcome to apply
  • CPR certification required upon hire or to be obtained during orientation, with maintenance of current certification throughout employment
Specific Job Responsibilities
  • Engage in community health initiatives as appropriate
  • Proficient in Microsoft Word, Excel, and email communication
  • Collaborate with patients, families, and support departments to effectively address patient needs
  • Exhibit proper body mechanics consistently
  • Make independent decisions when necessary
  • Understand and comply with relevant laws, regulations, and guidelines in post-acute care
  • Implement and interpret departmental programs, goals, objectives, policies, and procedures
  • Maintain proficiency in all competency areas, including patient rights, safety, and sanitation
  • Foster professional relationships with colleagues and vendors
  • Ensure confidentiality of all proprietary and sensitive information
  • Adhere to company policies, including those related to harassment and compliance
  • Demonstrate integrity and professionalism by following the organization’s Code of Conduct and completing required compliance training
Essential Functions
  • Implement physical therapy treatment plans for patients
  • Establish, assess, and adjust realistic, measurable, and functional goals
  • Oversee and evaluate care provided by PTAs, Rehab Aides, and PT and PTA students
  • Document patient care accurately and in a timely manner
  • Utilize therapy software effectively
  • Provide excellent customer service and maintain a positive attitude towards patients
  • Assist in patient evacuation when necessary
  • Demonstrate reliable attendance
  • Employ reasoning skills and sound judgment in daily tasks
  • Communicate effectively and work collaboratively within an interdisciplinary team
  • Perform physical tasks such as sitting, standing, bending, lifting, pushing, pulling, stooping, walking, reaching, and moving intermittently during work hours
  • Read, write, speak, and comprehend the English language
