We are seeking a dedicated Physical Therapist (PT) for a full-time or PRN position, offering a competitive wage range of $43 - $55 depending on experience. New graduates are encouraged to apply!

Our facilities operate as Skilled Nursing Facilities (SNFs) , providing:
- Long-term care for residents needing ongoing skilled support and compassionate daily assistance
- Short-term, sub-acute rehabilitation for patients recovering from surgery, illness, or injury
- Collaborative care through an interdisciplinary team approach involving nursing, therapy, and medical staff
- A diverse patient population with orthopedic, neurological, cardiopulmonary, and post-acute needs
As a therapy professional, you will play a crucial role in helping patients achieve functional improvements and enhance their quality of life in a supportive, patient-centered setting.
Position SummaryThe Physical Therapist will deliver direct rehabilitation care in accordance with physician referrals, adhering to all relevant laws, regulations, and organizational standards.
Qualifications and Requirements- Graduation from an accredited physical therapy program (BSPT, MSPT, or DPT; those graduating in 2003 or later must hold an MSPT or DPT)
- Current registration/licensure in the applicable state, with an active license in good standing throughout employment
- Preferred one year of experience; however, new graduates are welcome to apply
- CPR certification required upon hire or to be obtained during orientation, with maintenance of current certification throughout employment
- Engage in community health initiatives as appropriate
- Proficient in Microsoft Word, Excel, and email communication
- Collaborate with patients, families, and support departments to effectively address patient needs
- Exhibit proper body mechanics consistently
- Make independent decisions when necessary
- Understand and comply with relevant laws, regulations, and guidelines in post-acute care
- Implement and interpret departmental programs, goals, objectives, policies, and procedures
- Maintain proficiency in all competency areas, including patient rights, safety, and sanitation
- Foster professional relationships with colleagues and vendors
- Ensure confidentiality of all proprietary and sensitive information
- Adhere to company policies, including those related to harassment and compliance
- Demonstrate integrity and professionalism by following the organization’s Code of Conduct and completing required compliance training
- Implement physical therapy treatment plans for patients
- Establish, assess, and adjust realistic, measurable, and functional goals
- Oversee and evaluate care provided by PTAs, Rehab Aides, and PT and PTA students
- Document patient care accurately and in a timely manner
- Utilize therapy software effectively
- Provide excellent customer service and maintain a positive attitude towards patients
- Assist in patient evacuation when necessary
- Demonstrate reliable attendance
- Employ reasoning skills and sound judgment in daily tasks
- Communicate effectively and work collaboratively within an interdisciplinary team
- Perform physical tasks such as sitting, standing, bending, lifting, pushing, pulling, stooping, walking, reaching, and moving intermittently during work hours
- Read, write, speak, and comprehend the English language
