About Us The NC Department of Adult Correction is one of the largest state agencies with more than 14,000 employees. It is responsible for safeguarding and preserving lives and property through prevention, protection, and preparation as well as the care, custody, and supervision of all adults sentenced for violating North Carolina laws. This department includes state prisons with custody, healthcare, and facility operations staff as well as probation/parole officers who supervised sentenced people in the community. Other divisions include Administration, Health Services, Rehabilitation & Reentry, Special Operations and the Post-Release Supervision & Parole Commission. We have a mission that matters! Description of Work The Triage Charge Nurse provides comprehensive clinical care through assessment, intervention, coordination, and education to promote offender health and safety. Key duties include conducting telehealth triage via telephone or video consultations, evaluating medical complaints, and formulating timely nursing responses. The nurse performs accurate clinical assessments by integrating subjective symptoms with objective findings, applying sound nursing judgment, and operating independently within the scope of the Nurse Practice Act. This position supports continuity of care by developing and implementing care plans based on established protocols, provider guidance, and facility policy. It also includes assisting with urgent care, administering medications, coordinating follow-up care, responding to medical and mental health emergencies, and providing education on chronic disease, communicable conditions, treatment compliance, and preventive health measures. The nurse collaborates closely with providers, dental, and behavioral health teams, and follows through on clinical recommendations. The role requires the ability to think critically, make evidence-based decisions, and maintain a compassionate and professional demeanor. The Triage Nurse must foster trust with patients and contribute positively to interdisciplinary teamwork. The Triage Charge Nurse holds significant administrative and leadership responsibilities to support the delivery of quality care across multiple sites. Core duties include documenting and reviewing triage encounters and clinical actions, maintaining accurate and up-to-date nursing records, and generating routine reports related to clinical volume, case types, and performance metrics. The Triage Charge Nurse will provide education and training for the triage team, report clinical concerns to the appropriate clinical chains of command and provide updates and guidance to custody personnel. As a clinical leader, the Triage Nurse assists in orienting and mentoring nursing staff, delivering on-the-job training, and supporting quality assurance initiatives. The nurse serves as a resource for clinical decision-making, delegates appropriately, and communicates effectively with staff, management, and custody and medical personnel. This role requires strong organizational skills, a systematic approach to service delivery, and the ability to prioritize under pressure. The nurse must model professionalism, provide compassionate support to patients and families, and contribute meaningfully to staff development and interdisciplinary care planning. Position will be required to work any shift, weekends, or holidays as needed with limited supervision. This role is primarily remote; however, occasional onsite work, mandatory facility orientation, and travel to correctional facilities may be required based on orientation needs, program, and operational needs.
