Registered Nurse Case Manager - Specialty Surgery
The Registered Nurse (RN) Case Manager specializing in Specialty Surgery provides comprehensive, patient-centered care in an outpatient setting. This role involves assessing, planning, implementing, and evaluating nursing care, administering medications and treatments, performing clinical procedures, and providing patient education and care coordination in collaboration with healthcare providers and interdisciplinary teams.
Shift Information
Monday to Friday, 8:00 AM to 5:00 PM
Minimum Experience Required
1 year of experience
Certifications and Requirements
- Basic Life Support (BLS) certification
- Case Management certification preferred
Education Requirements
- Current, unrestricted Registered Nursing license in Alaska or another U.S. state
- Graduate of an accredited nursing program; Bachelor's degree preferred
- High school diploma or equivalent
Key Responsibilities
- Conduct comprehensive patient assessments to develop individualized care plans.
- Implement and evaluate nursing interventions based on patient responses.
- Administer medications and treatments safely and according to established guidelines.
- Recognize changes in patient conditions and initiate appropriate interventions or escalate care as needed.
- Deliver culturally responsive, patient-centered care.
Nursing Judgment and Clinical Decision-Making
The RN will apply critical thinking and professional judgment to:
- Perform and interpret assessments, recognizing significant changes in patient conditions.
- Exercise independent nursing judgment to determine appropriate nursing actions and priorities.
- Develop, implement, and revise individualized nursing care plans based on patient needs.
- Consider ethical principles and cultural considerations in nursing decisions, ensuring patient safety and compliance.
- Maintain accountability for nursing decisions and delegated tasks.
Care Coordination and Documentation
- Collaborate with providers and team members to coordinate care.
- Facilitate referrals, follow-up care, and continuity of services.
- Document all assessments, interventions, and outcomes accurately and in a timely manner.
- Advocate for patients and assist them in navigating the healthcare system.
Quality, Safety, and Compliance
- Adhere to regulatory and safety standards, including infection prevention.
- Participate in quality improvement initiatives.
- Report adverse events and unsafe conditions following established policies.
Teamwork and Professional Practice
- Exhibit professionalism, accountability, and flexibility in all interactions.
- Guide and delegate tasks to ancillary staff effectively.
- Support the orientation, mentoring, and education of staff and students.
