Home Health Physical Therapist - Greenwich, CT | $100K-$115K Annual Salary We are seeking a compassionate and skilled Physical Therapist to join our home health care team in Greenwich, Connecticut. In this rewarding role, you will deliver personalized physical therapy services directly to patients in the comfort of their own homes, helping them achieve improved mobility, independence, and quality of life. Key Responsibilities Conduct comprehensive physical therapy assessments and evaluations of patients in their home environment Develop and implement individualized treatment plans based on patient needs, goals, and medical conditions Provide hands‑on physical therapy interventions including therapeutic exercises, manual therapy, gait training, and mobility assistance Collaborate with physicians, nurses, occupational therapists, and other healthcare professionals to ensure coordinated patient care Educate patients and family members on proper therapy techniques, home exercise programs, and adaptive strategies Document patient progress, treatment outcomes, and medical records in accordance with regulatory standards Monitor and adjust treatment plans based on patient response and recovery progress Ensure compliance with all home health care regulations and quality standards Required Qualifications Current and valid Physical Therapist license in the state of Connecticut Doctor of Physical Therapy (DPT) or equivalent degree from an accredited program Minimum of one (1) year of clinical experience as a Physical Therapist Strong clinical assessment and treatment planning skills Excellent verbal and written communication abilities Ability to work independently with minimal supervision in a home care setting Strong organizational skills and ability to manage multiple patients and schedules effectively Valid driver's license and reliable transportation for home visits CPR certification (or willingness to obtain) Preferred Qualifications Previous home health physical therapy experience Experience working with geriatric populations Bilingual capabilities (Spanish/English) About Greenwich, CT Greenwich is a picturesque coastal town located in Fairfield County, Connecticut, known for its exceptional quality of life, beautiful waterfront properties, and vibrant community.
