Location: Portland, OR
Type: Contract
Shift & Schedule: Day Shift | Monday–Friday | 8:00 AM – 4:30 PM
Pay Rate: $1400 to $2000 Weekly (Based on experience)
Position Overview
We are seeking a compassionate Licensed Master Social Worker (LMSW) to provide home health services to patients throughout East Portland and the surrounding areas. The ideal candidate will have skilled home health experience, OASIS proficiency, and the ability to coordinate care while supporting patients and families in achieving optimal health outcomes.
Responsibilities
Conduct comprehensive psychosocial assessments for home health patients.
Develop and implement individualized care plans in collaboration with the interdisciplinary healthcare team.
Provide counseling, education, and community resource referrals to patients and their families.
Complete accurate OASIS documentation and maintain timely patient records in EPIC and related systems.
Coordinate discharge planning and connect patients with appropriate community services.
Travel to patient homes while managing an efficient daily schedule of up to six patient visits.
Ensure compliance with agency policies, regulatory requirements, and home health standards of care.
Required Qualifications
Master's degree in Social Work (MSW).
Current Licensed Master Social Worker (LMSW) license or eligibility for Oregon licensure.
Minimum of 1 year of skilled home health experience.
Current BLS (Basic Life Support) certification.
OASIS experience required.
Experience with EPIC or similar electronic medical record systems.
Valid driver's license, reliable personal transportation, and ability to travel throughout the service area.
COVID-19 primary vaccination series plus one booster, as required by the facility.
Preferred Qualifications
Experience working in a Home Health setting.
Strong communication, care coordination, and case management skills.
$42 - $52 per hour
