Hospice RN Admissions: Home Visits & Care Coordination
Visiting Nurse Association of Texas
A healthcare organization in Dallas seeks an RN Admissions to perform assessments and provide nursing care at patients' residences. This role requires coordinating care and collaborating with an interdisciplinary team to ensure seamless transitions into hospice services. Candidates must hold a current Texas RN license and have a minimum of two years in clinical care and one-year hospice experience.
