Care Coordinator Regular Full Time New Vista Newtown Clinic, Lexington, KY, US The Care Coordinator will be responsible for linking clients to medical services, assisting clients with navigation of medical services, assisting with addressing barriers to care for both behavioral health and medical care, monitoring client’s use of appropriate medical services, and following up with clients on issues relevant to receiving proper medical care. This position will be providing services in the clinic, in the community, and in client homes. Must have knowledge about the medical care system or have a relevant background to be able to attain such knowledge. Required Education and Experience Bachelor’s degree in Psychology, Social Work, Public Health or related field Valid driver’s license and proof of insurance Preferred Education and Experience Experience providing case management services Master’s degree in Psychology, Social Work, Public Health or related field Essential Functions Reasonable accommodations may be made to enable individuals with disabilities to perform the essential functions.
