Home Care Physical Therapist

Liberty Health is seeking a Home Care Physical Therapist to provide person-centered therapy services to adults in their homes. In this role, you will evaluate functional mobility, develop evidence-based treatment plans, and deliver interventions that promote safety, independence, and quality of life. You will collaborate closely with an interdisciplinary team, document outcomes, and educate patients and caregivers. This position offers the opportunity to support diverse populations, contribute to data-driven care, and improve community health outcomes through individualized, compassionate home-based therapy. Provide in-home physical therapy evaluations and treatments Develop and implement individualized, evidence-based care plans Assess functional mobility, strength, balance, and safety risks Educate patients and caregivers on exercises, equipment, and safety Collaborate with an interdisciplinary team on coordinated care Document assessments, progress notes, and outcomes accurately Adjust treatment plans based on patient response and goals Communicate effectively with referring providers and care partners Promote independence and quality of life in home settings Ensure compliance with professional, ethical, and regulatory standards Physical therapy evaluation and treatment Home health care practices Gait and balance training Therapeutic exercise prescription Patient and caregiver education Electronic health record (EHR) documentation Care plan development Interdisciplinary team collaboration Functional mobility assessment Fall risk assessment and prevention Medical Insurance Dental Insurance Vision Insurance 401k Current state Physical Therapist (PT) license Doctor of Physical Therapy (DPT) preferred BLS/CPR certification Valid driver’s license and reliable transportation Experience with home health or community-based care a plus #J-18808-Ljbffr Reporter Newspapers
