Minimum Requirements

• Bachelor‘s, Master‘s or Doctoral Degree in Physical Therapy from accredited schools approved by the APTA
• Active State License is Required to Start the Assignment
• BLS Certification May Be Required from AHA or ARC
• Experience in Home Health is required
• Lymphedema certification is a plus

Assignment Details

• Facility Type: Home Health
• Shift Name: Days
• Points per visit: 6 points/day (regular visit=1 pt, start of care=2.5 pts, eval 1.5 pts)
• EMR: Homecare Homebase
• Mileage reimbursement: $0.50/mile from first visit to last visit
