Job Description

Take your Nurse Practitioner expertise into the community with a flexible, per diem opportunity focusing on comprehensive home health risk assessments. This position supports Medicare Advantage members in San Miguel County, serving areas such as Las Vegas, Pecos, and Montezuma.

Schedule/Hours:

• Flexible scheduling with shifts available any day between Monday and Sunday
• Choose your own hours
• Commitment to a minimum number of hours per week or every other week

Key Responsibilities:

• Perform annual home health risk assessments for Medicare Advantage plan participants in their homes
• Assess prior and current medical history, medical conditions, and medication usage
• Deliver education and guidance to patients regarding their conditions and medications
• Identify gaps in care and provide clear recommendations for follow-up
• No prescription writing or creation of treatment plans is needed

Qualifications:

• Active Nurse Practitioner license in New Mexico
• Board certification as an NP through AANP or ANCC
• Current CPR (BLS) certification
• MSN degree from an accredited nursing school
• Preferably, at least six months of current Nurse Practitioner experience

Enjoy pay rates between $80-$90 for each completed evaluation, and receive additional compensation for mileage and administrative time.
