The RN Case Manager will serve as primary contact person and coordinator for adults with disabilities in a group home setting in Berkshire County.

Perform heath care assessments and develop written health care plans
Consult with program staff on specific medical interventions, techniques and documentation requirements.

Current Registered Nurse license with at least one year experience. Some experience with people with developmental challenges preferred.
Valid driver's license & Insured Vehicle
