Physical Therapist

Job Overview The Physical / Occupational Therapist evaluates, plans, and delivers individualized therapy programs in clients' homes. Reporting to the Home Health Services Director, this role focuses on improving patient mobility, restoring functional daily living skills, and training caregivers to foster independence and safety. Key Responsibilities Assessments & Plans: Conduct therapy evaluations and build tailored treatment plans based on client goals. Therapy & Training: Deliver rehabilitation exercises, train clients/caregivers on adaptive equipment, and promote safety. Care Coordination: Document progress, update care plans, and collaborate with physicians, nurses, and the care team. Compliance: Maintain timely, accurate clinical records in line with state/federal healthcare regulations. Qualifications & Demands Accredited Degree in Physical Therapy (PT) or Occupational Therapy (OT) with an active State License. CPR/First Aid certification; previous home health or clinical experience preferred.
