Responsibilities
- Manage discharge planning and care coordination from admission through transition to the next level of care, including needs assessment and coordination of DME and services.
- Collaborate with the interdisciplinary team (nursing, physicians, social services, and ancillary staff) to develop and implement the plan of care and ensure timely disposition.
- Conduct patient assessments for discharge readiness, identify barriers to discharge, and coordinate referrals and post-acute care services as needed.
- Document care plans, assessments, and transition activities in the EPIC charting system; maintain accurate, timely records.
- Provide cross-unit case management across Med-Surg, LLOS, and ED as assigned; float to other units as needed to support patient flow.
- Communicate clearly with patients and families about care options, expected outcomes, and follow-up plans.

Requirements
- Active RN license in Washington state or eligibility for licensure with pending license accepted
- BLS certification
- Minimum 1 year of acute care experience
- Discharge planning and care coordination experience
- Case management experience across Med-Surg, LLOS, and ED preferred
- EPIC charting experience required
- Ability to float across units and campuses as needed
- References: 2 references covering 1 year within the last 3 years required
- Pre-start modules (~16 hours) and orientation (~30 hours) required
