Job Summary The primary function is to administer skilled nursing care, under thesupervision of a registered nurse, for clients of all ages in their place ofresidence, coordinate care with a multi-disciplinary team, patient/family, andreferring agency. Essential Functions Demonstratesknowledge and competency in performing baseline nursing skills as outlinesin Agency policy and clinical policy; maintains clinical competenciesaccording to home care standards; complies with state practice act. Identifies anddetermines priority of the patient’s need/problems, anticipatesemergencies, complications of pathology and assists others in implementingcomplex care. Administers allmedications and treatments according to Agency policy, accepted standardsof care and home care guidelines. Communicateseffectively with the RN, patients, families, and physicians; documentspatient status, patient needs/problems, nursing interventions/patientresponse, safety interventions and education/teaching. Completes andsubmits documentation timely and accurately according to Agency policy. Provides careunder the supervision of the RN according to the plan of care establishedby the physician. Under thedirection of the RN, supplements the nursing care needs of the patient asprovided by the RN and as specified in the POC. Completeappropriate clinical and progress notes, documenting outcomes ofinterventions. Provide servicesin accordance with organizational policies and procedures and regulationsthat define scope of practice. Assist the RNand/or physician in performing specialized duties related to end-of-lifecare. Prepare equipmentand materials for treatment adhering to aseptic technique as required. Assesspatient/family response to care, ensuring communication of informationappropriate team members. Promote Agencyphilosophy and administrative policies to ensure quality of care. Maintains patientrights and dignity. Maintains acommitted and cooperative attitude with staffing, promotion of teamworkand harmony by readily assisting other staff and patients; participates incase conferences/team meetings. Makes visits asscheduled and participates in on-call rotation as required. Additional Responsibilities Performs relatedduties as assigned or requested. Conforms to allapplicable Agency policies and procedures. Participatesactively in continuing education and in-services. Maintainsconfidentiality of patient information and business trade practices Assumesaccountability for reporting incidents and complaints according to Agency policy. Time management Cooperativeattitude Basic math skillsrelated to patient care. Keeps abreast ofchanges in health care law. Maintainsagency/program compliance with local, state, and federal laws as well as stateaccreditation standards. Minimum Position Qualifications Education:Graduate of an accredited School of Nursing for LVN or LPN Experience: Oneyear of experience in nursing Training:Appropriate competency assessments completed License / Certification: Current State LVN / LPN license; current CPR; current driver’s licenseand proof of auto liability Environmental Conditions Works in patient's homein various conditions, possible exposure to blood and body fluids andinfectious diseases. Ability to work flexible schedule, ability to travel locally,some exposure to unpleasant weather. Moderatenoise level; tasks may involve moderate stress and emotional demands. Physical Requirements Prolonged standing andwalking required. Ability to move up to 50 lbs. and move patients. Requiresworking under some stressful conditions to meet deadlines, to identify patientneeds, to make quick decisions and resource acquisition; meet patient/familypsychosocial needs. Requires hand-eye coordination and manual dexterity.Ability to utilize durable medical equipment in the home.
