LTSS Service Coordinator-Clinician (Indianapolis, IN)

Position Title LTSS Service Coordinator-Clinician (Indianapolis, IN) Schedule & Location Monday-Friday 8am-5pm EST. Candidates must be located in Marion County. This field-based role requires travel to client sites or designated locations as needed, with occasional office attendance for meetings or training. Responsibilities Assist responsible RN/LSW in identifying members for high risk complications. Obtain clinical data as directed by the responsible RN/LSW. Assist RN/LSW in identifying members who would benefit from alternative level of care or other waiver programs. Provide collected information to RN/LSW for verification and interpretation. Participate in coordinating care for members with chronic illnesses, comorbidities, and/or disabilities, ensuring cost‑effective utilization of health benefits. Minimum Requirements Licensed Social Worker (LSW, LCSW, LMSW) or equivalent non‑RN license in applicable state. Minimum 2 years of experience working with individuals with chronic illnesses, comorbidities, and/or disabilities in a Service Coordinator or similar role, or an equivalent combination of education and experience. Nursing Diploma or AS in Nursing or related field (if applicable). Preferred Skills & Experience MA/MS in Health/Nursing. State‑specified certification based on state law and/or contract. Willingness to travel to work sites and other locations as necessary.
