Base pay range $130,000.00/yr - $160,000.00/yr What you will do as a Field Nurse: Provide hands‑on patient care within RN scope (PICC lines, wound care, NG/G/J tubes, trach and ostomy care, IV/TPN, injections, blood draws, etc.) Support the Case Management RN in developing and executing patient care plans. Document accurately and timely within 24–48 hours, reflecting the plan of care and patient outcomes. Educate patients and families on diagnosis and care plans. Assist in skilled procedures and coordinate referrals to other disciplines. Maintain professionalism, privacy, and patient safety. Participate in on‑call duties, case conferences, and continuous growth opportunities. To be successful as a Field Nurse: 1 year of recent nursing experience (internal or new‑grad waiver possible). 1 year of home‑health experience preferred; high‑acuity care an asset. Graduate of an accredited nursing program (Bachelor’s required). Current California RN license and Basic Life Support (BLS) certification. Valid California driver’s license with acceptable record. Current, compliant with all licensing and certification requirements. Strong critical thinking, interpersonal communication, organization, and computer literacy. Commitment to ongoing compliance training and certifications. Compensation & Benefits Estimated wage range: $130,000 – $160,000.
