Social Service Director

Title: Social Service Director Department: Social Services/Administration Location: Symphony Care Network Status: Full Time Symphony Applewood is seeking a compassionate and dedicated Social Service Director to support our short-term rehab and long-term care guests. This role is responsible for providing psychosocial support, discharge planning, advocacy, and care coordination while working closely with residents, families, and the interdisciplinary team.
