Position Highlights: Monday–Friday schedule, typically 8:00 a.m. to 5:00 p.m. Flexible scheduling options Interdisciplinary team meetings held every other Wednesday Competitive compensation based on experience Supportive team environment focused on collaboration and patient-centered care Responsibilities Include: Assessing patients' physical, emotional, and psychosocial needs Educating patients, families, and caregivers on disease progression and care plans Managing pain and symptom control to maximize comfort and quality of life Coordinating care and serving as the primary case manager for assigned patients Collaborating with an interdisciplinary team that includes physicians, social workers, chaplains, CNAs, therapists, and volunteers As a Hospice RN Case Manager, you will work under the direction of the Medical Director and Clinical Director while building meaningful relationships with patients and families during a critical stage of life.
