Doctor of Pharmacy (PharmD) degree, active Pharmacist license, and minimum 1+ years Pharmacist experience required. Applicants who do not meet these qualifications will not be considered.
A clinical pharmacy opportunity is available for a travel assignment at a Long-Term Acute Care (LTAC) facility with a 13-week duration, starting ASAP. This role focuses on hospital-inpatient pharmacy services within a dynamic healthcare setting, emphasizing both clinical expertise and operational pharmacy management.
Key Responsibilities:
- Maintain adequate drug inventory; conduct annual inventory and handle procurement, including emergency orders.
- Ensure compliance with all federal and state pharmacy regulations, including controlled substances management.
- Prepare, label, and dispense medications accurately, including sterile and parenteral preparations.
- Conduct monthly inspections of pharmacy and patient care units to ensure proper drug storage and handling.
- Collaborate on hospital formulary development and staff education; participate in Pharmacy and Therapeutics Committee meetings.
- Provide clinical consultation, medication reconciliation, discharge counseling, and monitor patient drug therapies for safety and effectiveness.
- Utilize EPIC systems for medication reconciliation and billing functions.
- Support antimicrobial stewardship and pharmacokinetic dosing including anticoagulation management.
- Develop staff through training, evaluation, and positive team leadership.
Qualifications & Desired Experience:
- Previous experience working in a hospital or LTAC pharmacy setting.
- Basic Life Support (BLS) certification.
- Strong clinical knowledge in antimicrobial stewardship, pharmacokinetic dosing, and anticoagulation.
- Effective communication skills for patient counseling and interdisciplinary collaboration.
- Familiarity with EPIC electronic medical records system.
- Ability to manage medication reconciliation processes, including gathering accurate home medication lists and discharge counseling.
