The Rehabilitation Counselor provides comprehensive counseling, care coordination, advocacy, and community-based support to individuals experiencing disabilities, mental health concerns, vocational barriers, or other challenges affecting independent living and successful community integration. The counselor utilizes strong interpersonal and communication skills to establish rapport, assess individual needs, develop goal-oriented service plans, and connect clients with appropriate clinical, vocational, educational, and community resources.

The ideal candidate is highly articulate, approachable, resourceful, and proactive in outreach. This professional effectively communicates with clients, families, employers, healthcare providers, community organizations, and other stakeholders to promote access to services and support individuals in achieving greater independence and improved quality of life.

A successful Rehabilitation Counselor will serve as both a counselor and community connector , combining therapeutic communication, advocacy, resource coordination, and outreach to help individuals identify meaningful goals and overcome barriers to achieving them. The position requires a professional who can establish trust with clients while simultaneously developing strong partnerships throughout the community to expand access to services, opportunities, and supports.
