Acute Care Social Worker: Discharge & Transitions Lead

St. Mary Medical Center (Pennsylvania)

Saint Francis Hospital in Wilmington, DE is seeking a Social Worker - Acute Care to monitor patient progress, coordinate discharge planning, and document findings for quality care and efficient transitions. The role requires an MSW and at least 1 year of acute care experience, with strong communication and leadership skills.
