VibrantCare Rehab is seeking a dedicated Physical Therapist (PT) to join our team in Castro Valley, CA. The Physical Therapist will provide high-quality, patient-centered care while collaborating closely with patients, caregivers, physicians, and facility staff to promote optimal rehabilitation outcomes.

 

Responsibilities

• Evaluate and treat patients requiring skilled physical therapy services.
• Develop individualized treatment plans and monitor patient progress.
• Document evaluations, treatments, goals, progress, and medical necessity accurately and timely.
• Supervise and direct Physical Therapist Assistants and aides as appropriate.
• Communicate effectively with patients, families, physicians, coworkers, and facility staff.
• Participate in facility meetings, case management, and clinical in-services.
• Maintain compliance with state regulations, Medicare requirements, OSHA standards, and company policies.
• Maintain required continuing education and CPR certification.
• Support positive relationships with facilities, clients, and healthcare partners.
• Meet productivity and quality-of-care expectations.

 

Required Qualifications

• Graduate of a CAPTE-accredited Physical Therapy program or substantially equivalent international education.
• Current California Physical Therapist license/registration as required.
• Current CPR certification as required.
• Strong clinical problem-solving and communication skills.
• Excellent organizational, time-management, and planning skills.
• Ability to work effectively as part of a team.
• Ability to supervise and support PT assistants and aides.
• Strong computer and clinical documentation skills.

 

Compensation & Benefits

• Base Salary: $85,000–$95,000/year (Based on Experience)
